Form
In order to prevent discrepancies and other issues, Mama advises all customers to complete the following inventory form.  While it is not required, it is heavily suggested.

After receiving your laundry pick-up, Mama verifies each item on this form.  If there are any discrepancies, Mama won't proceed with your order until the issue is resolved.  While it may seem time consuming to both parties, it helps ensure your satisfaction.  And Mama wants you to be satisfied.

You may be as specific or as general on the form as you like.

For example:
  • Long-Sleeved Shirts: 2
OR
  • Long-Sleeved Shirts: 2 - 1 white and 1 striped

This form is also the designated place to write in any special care instructions for your garments - hang dry, wash in mesh bag and then hang dry, dry on low heat, etc.

Also use this form to notify Mama of any known stain issues.  While Mama looks clothes over quickly for stains, it is more efficient to eliminate stains if Mama knows of them ahead of time.  Document on this form.  If you feel like going the extra mile, it would be fabulous if you put a safety pin next to the stain on your garment.
